The State of Missouri to: Eddie Chamberlain and John Doe
You are hereby notified that a Petition to Terminate Parental Rights, for Transfer of Custody, and for Adoption has been commenced in the Circuit Court of Daviess County, the object and general nature of which is to terminate your parental rights, transfer custody of your child to the Petitioner’s, and allow Petitioner’s to adopt the child.
The names of all parties to said suit are stated above in the caption hereof and the name and address of the attorney for Petitioner is: Annie Gibson, 109 E. Jackson, Gallatin, MO 64640.
You are further notified that, unless you file an answer or other pleading or shall otherwise appear and defend against the aforesaid Petition within forty-five (45) days after the 7th day of December, 2005, judgment by default will be rendered against you and an Order will be rendered in accordance with the Petition filed herein.
It is ordered that a copy hereof be published according to law in a newspaper of general circulation published in Daviess County, Missouri.
