Jack L. Hunsucker, Administrator of the Daviess County Health Department announces that the “County-wide Health Needs Survey” will soon be mailed to a random sample of Daviess County households. This questionnaire was developed in collaboration with Grundy, Livingston, Caldwell, Harrison and Tri-County (Worth, Gentry & DeKalb) Health Departments as part of the Northwest Regional Health Assessment being conducted in cooperation with Truman State University.
The goal of each county public health department is to work together with their communities to improve health services of county residents. Responses to this questionnaire will help each health department to determine what services to continue, add or improve. All questionnaires will include a postage-paid reply envelope. All responses will be held in strictest confidence and analyzed as a regional group and by individual county. Truman State University is providing faculty and student expertise for mailing, final analysis and report.
The Daviess County Health Department staff and elected Board of Trustees strongly encourage all residents who receive this important questionnaire to take a few minutes to complete and mail back the survey. Any comments or questions may be directed to the Daviess County Health Department , 660-663-2414.
