July 10 – 8:30 a.m. – noon — 12:30 – 3:45 p.m.
July 31 – 8:30 a.m. – noon — 12:30 – 3:45 p.m.
August 14th – 8:30 a.m. – noon — 12:30 – 3:45 p.m.
Cost: $10 Donation is asked to cover costs or covered on Medicaid annually
Parent must accompany child to fill out paperwork • Need to bring shot records
Allow at least one hour for this appointment
To ensure good clinic flow anyone 15 minutes late will be rescheduled.
