The Daviess County Health Department at 609A South Main in Gallatin will be offering sports physicals and E.P.S.D.T.’s on June 13, July 11, July 18; Aug. 8 and Aug. 15 from 1-4 p.m. each day. Appointments are limited. Call for an appointment at 660-663-2414.
E.P.S.D.T. is an advanced physical that is covered by Medicaid. Parents need to accompany the child to sign permission forms and physical forms. Please bring the child’s immunization record. A $10 donation is asked to cover costs. The physicals and E.P.S.D.T.’s are for school age children only.
