Notice of Corporate Dissolution

NOTICE OF CORPORATE DISSOLUTION TO ALL CREDITORS AND CLAIMANTS AGAINST DAVIESS COUNTY CARE CENTER, INC.

Notice is hereby given that Daviess County Care Center, Inc., a Missouri corporation, with corporate offices at Highway 6 West, Gallatin, Missouri 64640, is being liquidated and dissolved pursuant to the Missouri Non-Profit Corporation Act. This notice is being given pursuant to Section 355.701 RSMo., of The Missouri Non-Profit Corporation Act.

1. Articles of Dissolution on behalf of the Corporation were filed with the Missouri Secretary of State on January 31, 2000 and the effective date of dissolution was January 31, 2000.

2. Claims against the Corporation must include:

a. The name, address and telephone number of the claimant.

b. The amount of the claim.

c. The date on which the event on which the claim is based occurred.

d. A brief description of the nature of the debt or the basis for the claim.

3. The mailing address where claims against the Corporation must be sent is: Daviess County Care Center, Inc., c/o Robert Cowherd, Attorney at Law, P.O. Box 228, Chillicothe, Missouri 64601.

4. All claims against Daviess County Care Center, Inc. will be barred unless the proceeding to enforce the claim is commenced within two (2) years after the publication of this notice.
